Waiver & Release
On behalf of myself and my minor child, hereby consent to allow my child(ren) to be photographed by AnetaArt, Inc., or by any of its officers, employees, or other agents. I understand and acknowledge that any such photos may be used for advertising, publicity, commercial purposes, or for any other reason. I further consent to all such photos being placed on the Internet, such as on social media accounts operated by AnetaArt, Inc. I consent to all such photos being edited, cropped, and otherwise altered as AnetaArt, Inc. sees fit. I understand that I have no right or interest in any of these photos. I hereby forever release AnetaArt, Inc. – and by all of its officers, employees, and other agents – from all claims of every kind on account of using the photos. This release includes, without limitation, any claims for invasion of privacy, right of publicity misappropriation, and defamation. I acknowledge that third parties may be able to download any of the photos that are placed online. The photos may be used in conjunction with the name of the location in which the activities took place. I understand that activities conducted by AnetaArt, Inc. may in involve the use of potentiallydangerous tools such as hot glue guns, sewing machines, and craft knives. Although AnetaArt will take reasonable precautions to prevent harm to students, the possibility for injuries still exist. I understand and accept these risks on behalf of myself and my minor child(ren). I further hereby release AnetaArt, Inc. – and all of its officers, employees, and other agents – from liability for injuries or death that that may occur, whether due the action, inaction, or negligence of my minor child(ren), or the action, inaction, or negligence of others. I also release from liability anyone who administers reasonable medical assistance to my minor child(ren). I will alert AnetaArt, Inc. of any medical issues that may impact my minor child(ren)’s participation in AnetaArt, Inc.’s activities In the sole discretion of AnetaArt, Inc., unsafe or disruptive behavior will be grounds for removing a child from class, or from future classes. I hereby warrant that I am at least eighteen (18) years old and competent to contract in the name of myself and my minor child(ren). This release is irrevocable and binding on me, my minor child(ren), and on any of our heirs, legatees, assigns, and personal representatives. The laws of the State of Illinois shall govern the validity and construction of this Agreement, regardless of any principles of conflicts of law.
